If your lower back has been aching for days, locking up when you bend, or sending a sharp line of pain down your leg, you already know how much it can take over your life. It changes how you sit, how you sleep, how you pick your kids up. Low back pain is the single most common reason people walk through my door, and here’s what I want you to know straight away: in most cases it gets better, often a lot better, once someone works out what’s actually causing it.
I’m Saurabh Patel (Sam), physiotherapist and owner of Back to Ease Physiotherapy. I hold a postgraduate degree in Advanced Clinical Musculoskeletal and Sports Physiotherapy, and the spine is something I work with week in, week out. But qualifications only tell part of the story. What changes outcomes is the time spent listening, assessing properly, and refusing to settle for a quick fix that fades in a fortnight.
What I care about isn’t just easing your pain this week. It’s understanding why the pain turned up in the first place – the movement pattern, the workload, the weakness – so we can address that and stop it cycling back months later. Treating the symptom alone is how people end up in that exhausting loop of feeling better, then flaring up again. I’d rather break the loop.

The first appointment is about understanding your story. I’ll ask how the pain started, what makes it better or worse, how it’s affecting your sleep and your day, and what you’re hoping to get back to. Then I’ll assess how your spine, hips and surrounding muscles are genuinely moving. By the end, you’ll have a clear explanation in plain language and a sense of the road ahead.
From there, treatment is usually a blend of two things. There’s the hands-on side – manual therapy and joint mobilisation to ease stiffness, and where it helps, dry needling for tight, guarded muscles. Then there’s the active side, where the lasting change happens: specific, progressive exercise that rebuilds your back’s tolerance so it can handle your real life again. You won’t be handed a generic photocopied sheet of stretches on your way out – everything is built around where you are now and where you want to be.
